GP Diversion Liaison Nurse - Acute Specialist Clinics - Care Synergy Project

About the role

As the GP Diversion Liaison Nurse, you will support the development, coordination, and implementation of GP diversion pathways in Acute Specialist Clinics as part of the CareSynergy Project. This role is key in providing the critical interface between clinical specialty teams and operational functions to enable seamless coordination and delivery of diversion pathways.

 

You will support the CareSynergy Project to deliver safe and timely clinical diversions to established GP partners. The GP Diversion Liaison Nurse will develop, manage and coordinate a centralised approach to workflow design, feedback processes, and referral mechanisms focused on improving coordination between hospital and primary care and promoting equitable and improved timely access.

 

About you

To be successful in this role you will possess:

  • Current Registered Nurse registration with AHPRA.
  • Demonstrated clinical skills aligned with experience and scope of practice.
  • Willingness to work collaboratively as part of a multi-disciplinary team, and contribute to the professional development of self and others.
  • High level communication, both written and verbal, with demonstrated high level interpersonal skills with patients, families and all health care professionals.
  • High level of motivation and willingness to learn, undertaking professional development to enhance nursing and midwifery expertise.
  • Demonstrated provision of high-quality care, that reflects the application of evidence into practice.
  • Digital literacy and embracing of health technologies and informatics.

 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
