Allied Health Assistant Grade 2 – GEM and Rehab@Home

About the role

We have an opportunity for a Grade 2 Allied Health Assistant to join our Care@Home program based at the Peter James Centre in Burwood East.

 

As part of the dynamic Geriatric Evaluation and Management (GEM) and Rehab@Home service within the At Home Program Directorate, you will play a key role in delivering hospital-level, patient-centred care in clients' homes. This service provides comprehensive multidisciplinary support, including Geriatrician/Rehabilitation Physician oversight, nursing, and allied health input, with patients receiving daily visits for approximately two weeks following discharge.

 

You will work with a diverse caseload across the eastern catchment, providing mobility and exercise programs, equipment set-up assistance, and functional retraining under the guidance of Physiotherapists and Occupational Therapists. Daily team meetings and multidisciplinary collaboration support strong engagement and high-quality patient care.

 

You will also have regular engagement with the broader Physiotherapy and Occupational Therapy departments to support your professional development. On-site Grade 3 senior clinicians provide supervision and guidance, offering opportunities to achieve your professional goals and contribute to quality improvement initiatives.

About you

To be successful in this role you will possess:

  • Completion of an Allied Health Assistant Training Qualification and a minimum of 2 years of experience post completion of course.
  • Full Victorian Driver's License with no restrictions.
  • Highly developed interpersonal skills and the ability to work autonomously.
  • A strong commitment to high-quality care.
  • The ability to work as part of a diverse team.   
  • Excellent communication and organisational skills.

 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
