About the role

 

 

As a program, our goal is to work in partnership with our clients to achieve the best possible outcomes. The Case Manager is a pivotal member of the Transition Care Program (TCP) multidisciplinary team. We provide our program for clients needing bed-based services at three residential sites within the Eastern Health catchment – and, via a community-based service for clients in their own home. This position will be based at one of our residential TCP sites.

 

 

The Case Manager draws upon their clinical knowledge and experience to work with clients and their families/carers and other members of the multidisciplinary team. They assist with the client’s transition back into the community or into a residential care facility of their choice. They facilitate collaboration between various teams and in the coordination and provision of services. The Case Manager does not provide daily care and nursing to clients, instead they assist clients and their family/carers with adjusting to changes in health status and facilitate timely discharge of clients from the program. Their role is particularly crucial in initiating processes for clients without family/carer support.

 

 

About you

 

 

To be successful in this role you should possess:

 

 

  • Previous relevant case management experience in either a health care setting or in the community.
  • Effective interpersonal skills and the ability to communicate with a broad range of internal and external customers.
  • Well established assessment skills and the ability to identify psycho-social issues.
  • Knowledge of and the ability to apply appropriate intervention strategies (e.g. conflict resolution, grief and loss)
  • A good knowledge of community resources within the region.
  • Demonstrated ability to work both autonomously and in a multi-disciplinary team.
  • Self-motivated with a demonstrated innovative approach to work practices and the ability to streamline and prioritise workload with the flexibility in moving between tasks at short notice.
  • A current Victorian Driver’s Licence and access to own vehicle.
  • A recognised tertiary qualification in a health related discipline that meets registration and/or professional body membership requirements.
