About the role

As a program, our goal is to work in partnership with our clients to achieve the best possible outcomes. The dietitian is a pivotal member of the Transition Care Program (TCP) multidisciplinary team. We provide our program for clients needing bed-based services at three residential sites within the Eastern Health catchment – and, via a community-based service for clients in their own home. The dietitian works with both the bed-based and home-based TCP clients and teams. The role of the dietitian is to draw upon their clinical knowledge and experience to work with clients and their families/carers and other members of the multidisciplinary team. Clinical activity for the dietitian on the team includes direct therapy services, as well as liaison with families, education and student supervision. There are also opportunities to be involved in quality improvement, research and service development activities with both the EH dietetic and TCP teams. 

 

About you

To be successful in this role you should possess:

  • Eligible for membership of the Dietetics Australia and the Accredited Practicing Dietitian Program.
  • Effective interpersonal skills and the ability to communicate with a broad range of internal and external customers.
  • Well established assessment skills.
  • A good knowledge of appropriate community resources.
  • Demonstrated ability to work both autonomously and in a multi-disciplinary team.
  • Self-motivated with a demonstrated innovative approach to work practices and the ability to streamline and prioritise workload with the flexibility in moving between tasks at short notice.
  • A current Victorian Driver’s Licence and access to own vehicle. EH fleet vehicles available.
