About the role
The Manager, Transition Care Program (TCP) provides operational leadership across both community-based and bed-based transition care services, ensuring high-quality, person-centred care for older people following hospital admission. Reporting to the Deputy Director of Operations, you will lead an interdisciplinary team and oversee service delivery that is safe, effective, and aligned with relevant legislation, standards and Eastern Health policy.
This role has a strong operational and systems focus, providing oversight of clinical governance, workforce management, financial performance, and contract oversight of associate providers. A key accountability is building and sustaining effective partnerships across internal teams and external providers, ensuring services meet agreed quality, performance and compliance expectations, including alignment with the Strengthened Aged Care Quality Standards. You will play a critical role in supporting system-wide patient access and flow, driving timely intake, coordinated care planning and efficient discharge pathways.
Equally important is your role in developing the workforce and shaping a positive, values-based culture. You will lead workforce planning, capability development and performance frameworks, ensuring staff are supported, engaged, and equipped to deliver safe, high-quality care, while fostering an environment of respect, collaboration and continuous improvement.
About you
To be successful in the role, you will possess:
• A clinical qualification with current AHPRA registration, or eligibility for membership of the relevant professional body.
• Demonstrated experience leading multidisciplinary teams in complex healthcare environments, including workforce planning, coaching, mentoring and performance management.
• Strong capability in building a positive, values-based workplace culture, with a focus on staff engagement, capability development and retention.
• Experience in contract management and working with associate providers, ensuring performance, compliance and high-quality service delivery.
• Expertise in clinical governance, quality, risk management and compliance, ideally within aged care or subacute services.
• Strong operational and financial management skills, including budget oversight and service sustainability.
• Experience driving patient access and flow improvements, including intake, service coordination and discharge planning.
• Strong analytical capability, with experience using data, benchmarking and performance metrics to drive improvement and decision-making.
• Proven ability to lead continuous improvement and change initiatives, using structured improvement methodologies.
• Demonstrated commitment to consumer partnership and co-design, working across the care continuum.
• Experience in Transition Care and/or aged care services, including understanding of contemporary reform and quality standards.
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
Those that are offered and/or start between January and April do not require an up to date vaccination for Influenza to progress providing they are aware of its requirement later in the year. Those that are offered and/or start between May and December require the up to date Influenza vaccination prior to offer.
