About the role 

This is an exciting opportunity for a dynamic and enthusiastic Grade 2 Occupational Therapist to join the Early Stroke Discharge (ESD) program at Eastern health.   

 The Early Stroke Discharge (ESD) service provides community based multidisciplinary care to support patients transitioning from bed-based services to their own homes. The ESD service delivers a 6 day a week service under both ambulatory and bed substitution models across 2 Eastern Health sites-Peter James Centre (Burwood East) and Angliss Community Rehabilitation Centre (Upper Ferntree Gully). ESD patients are seen in the home and/or in the centre. We are a small team of 14 staff members that service approximately 12-14 patients across the Eastern Health catchment area.  

This is a Monday to Friday position primarily based at Peter James Centre in Burwood but will also on occasions cover the Angliss Community Rehabilitation Centre in Upper Ferntree Gully. The role will also have the opportunity to rotate into the Community Rehabilitation Program at Peter James Centre. 

 

About you  

To be successful in this role you will possess 

  • Experience in the area of neurological rehabilitation 
  • Strong commitment to high quality patient care 
  • Clinical skills in managing a complex caseload using a client-centred approach 
  • High level communication skills, both written and oral, to enable development of a good rapport with clients and staff. 
  • The organisational skills of a self-starter able to administrate a defined caseload 
  • Demonstrated ability to participate in innovative practice and quality improvement 
  • Deliver a high quality, accessible and coordinated service to our clients both in the client’s home and in the centre. 
  • Ability to work collaboratively with other disciplines to achieve client-centred goals 
  • Ability to be flexible and change work priorities as required in a fast paced, dynamic service 
  • Demonstrated confidence and interest in teaching occupational therapy students on top of clinical caseload
