About the role
We have an exciting full-time opportunity for a Physiotherapist to join our Care at Home program, based at the Peter James Centre, following an internal promotion. As part of the dynamic GEM and Rehab@Home service within the at home program directorate, you will play a key role in delivering hospital-level, patient-centred care in the comfort of clients’ homes. This service provides comprehensive multidisciplinary support, including Geriatrician/Rehab Physician oversight, nursing and full allied health input, with patients receiving daily visits for approximately two weeks post-discharge. You will work with a highly varied caseload across the eastern catchment, offering significant versatility in the types of patients you see and the care required, giving you strong clinical breadth and autonomy in your day-to-day practice. Daily team meetings and collaborative sessions with the multidisciplinary team foster strong engagement, shared decision-making and consistently high-quality patient care. You’ll also have regular engagement with the broader Physiotherapy department to support your ongoing professional development. On-site Grade 3 senior clinicians provide supervision and guidance, and you’ll have opportunities to supervise Grade 1 staff, students and AHAs, as well as contribute to quality improvement activities. This is a fantastic opportunity to enhance your skills in communication, dynamic risk assessment, dignity of risk and person-centred goal setting while working within a supportive and highly engaged team.
About you
To be successful in this role you will possess:
- Recognised tertiary qualification in Physiotherapy
- Current registration with Physiotherapy Board of Australia (AHPRA)
- Demonstrated skills in the physiotherapy management of GEM and Rehabilitation caseloads
- Ability to meet all the requirements as outlined in the position description roles and responsibilities
- Ability to work autonomously given you will be working with patients in their own homes away from other staff members
- Full Victorian Driver’s License (with no restrictions)
- A strong commitment to high quality care
- The ability to work as part of a diverse team
- Excellent communication and organisational skills
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
