About the role

Provide service to clients in Bed based and Community Based Transition Care Program

The Grade 2 Physiotherapist plays a key role in delivering high-quality, evidence-based physiotherapy aimed at optimising functional independence and supporting safe discharge planning. In line with the expectations of a Grade 2 clinician, the physiotherapist provides therapy in an area requiring specialised clinical knowledge and depth of experience, particularly in the management of deconditioned, post-acute, or functionally limited older adults common within TCP.

 

The Grade 2 Physiotherapist works autonomously while contributing actively to the multidisciplinary team, ensuring timely assessments, goal-directed interventions, and clear communication with nursing, occupational therapy, case management, and broader TCP staff. They act as a clinical resource for the team, offering guidance to allied health assistants  and students, and supporting consistent, best-practice service delivery across the program.

As part of the TCP model, the physiotherapist is expected to:

  • Conduct comprehensive mobility, strength, and functional assessments to determine TCP functional goal.
  • Provide tailored excercise programme to maximise mobility, reduce falls risk, and support safe return to community living.
  • Liaise with families/carers and external services to ensure continuity of care.
  • Participate in case conferences and contribute to discharge planning.
  • Engage in ongoing  supervision, and quality activities, aligned with TCP’s focus on safe, effective, and person-centred care.
  • Represent physiotherapy within TCP when required by management.

The Grade 2 physiotherapist maintains up-to-date clinical knowledge and collaborates with peers to ensure consistent, high-standard service provision.

 

About you 

To be successful in this role you will possess:

Key Selection Criteria

• Recognised tertiary qualification in Physiotherapy

• Current registration with the Physiotherapy Board of Australia

• Eligibility for practicing membership of the Australian Physiotherapy Association

• Advanced clinical skills in managing a complex caseload and comprehensive discharge planning using a client-centred approach. • Broad clinical experience encompassing diverse patient presentation.

• Previous clinical experience working in an aligned clinical environment.

• High level communication skills, both written and oral.

• Demonstrated capacity to work / show leadership in a multidisciplinary team.

• Experience in supervision of staff and/or students.

• Demonstrated ability to introduce innovative practice.

• Commitment to improving own practice and undertaking quality and research activities.

• Demonstrated understanding of professional codes of conduct.

Desirable

• Participation in work practice coordination and review

• Previous experience working in TCP/Aged care settings.

• Experience in conflict resolution

• Working towards post graduate qualification would be highly regarded.

• Member, Australian Physiotherapy Association with current involvement in relevant special interest groups 

 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
