About the role
You will be part of a small social work team within the multidisciplinary GEM/REHAB@HOME service. The role consists of working with patients and their families regarding accessing community resources, adjustments to hospitalisation or illness, emotional wellbeing, financial/legal concerns and carer stress. It is an ever-changing dynamic role that requires you to work autonomously and liaise with patients both via telehealth and face to face in their own homes.
About you
To be successful in this role you will possess:
- Eligibility for membership of the AASW
- Evidence of current CPE, as per AASW requirements
- Current registration with AHPRA
- Highly developed interpersonal and leadership experience
- A strong commitment to high quality care and also work autonomously as required
- The ability to work as part of a diverse team
- Excellent risk assessment skills
- Excellent communication and organisational skills
- Conflict resolution skills
- The ability to lead complex discharge planning and understanding of community based services
- Current Victorian Drivers Licence
- Computer Literacy
Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
