About the Role


We have an exciting opportunity for an enthusiastic and skilled Social Worker to join our dynamic, multidisciplinary GEM@Home and Rehab@Home team at Eastern Health.

 

The GEM/REHAB@Home program has experienced rapid growth and is now one of the largest of its kind in the state. Our mission is to ensure that people with multiple and complex needs receive timely access to specialist assessment, diagnostic, and management services—including care coordination—within the comfort of their own home. This approach optimises recovery and reduces risks associated with hospitalisation.

 

As a Social Worker in this role, you will be part of a dedicated three-person social work team that also supports undergraduate social work students on placement. The work is diverse, fast-paced, and rewarding, offering opportunities to make a real difference in the lives of clients and their families.

 

About you 

 

  • Tertiary qualifications in Social Work and eligibility for membership of the AASW
  • Previous social work hospital experience  
  • Evidence of current CPE as per AASW requirements
  • Current Victoria driver’s license 
  • A strong commitment to high quality care
  • The ability to work effectively as part of a diverse team and also work autonomously
  • Excellent communication and organisational skills
  • Ability to work across a seven day a week roster 

 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
