About the role

 

This is an exciting opportunity for a dynamic and enthusiastic Grade 2 Allied Health Assistant to join the Early Stroke Discharge (ESD) program at Eastern health.   

 The Early Stroke Discharge (ESD) service provides community based multidisciplinary care to support stroke patients transitioning from bed-based services to their own homes. The ESD service delivers a 6 day a week service under both ambulatory and bed substitution models across 2 Eastern Health sites-Peter James Centre (Burwood East) and Angliss Community Rehabilitation Centre (Upper Ferntree Gully). ESD patients are seen in the home and/or in the centre. We are a small team of 14 staff members that service approximately 12-14 patients across the Eastern Health catchment area.  

This is a Monday to Friday position primarily based at Angliss Community Rehabilitation Centre in Upper Ferntree Gully but will on occasions provide cover at the Peter James Centre in Burwood. 

You will work closely with the multidisciplinary team to provide a flexible and responsive client centred service.  You will provide therapy support services to individual or groups under the delegation and supervision of an Allied Health Professional. There is also an administrative component to this role. 

 

 

About you 

 

To be successful in this role you will possess:

  • Completion of Certificate IV (Allied Health Assistant) from a registered training organisation, or recognised equivalent is essential. 
  • Highly developed interpersonal skills with can-do attitude 
  • A strong commitment to high quality care 
  • The ability to work as part of a diverse team and take direction from the team 
  • Excellent communication and organisational skills 
  • Deliver a high quality, accessible and coordinated service to our clients both in the client’s home and in the centre.  
  • Be an active and responsive member of the interdisciplinary team

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice).
