About the role
Join our dynamic multidisciplinary ASERT team and play a key role in delivering high-quality, evidence-based care that improves patient outcomes and supports smooth transitions from hospital to community.
As an ASERT Clinician, you will:
- Provide advanced clinical assessment and intervention.
- Collaborate closely with treating teams, patients, and community services to ensure timely, coordinated care.
- Contribute to process improvement, discharge planning, and patient flow.
- Act as a clinical resource and adviser within a supportive, multidisciplinary environment.
- Maintain up-to-date knowledge of best practice, community resources, and ethical standards.

About you
To be successful in this role you will possess:
Key selection Criteria
- Proven experience in complex clinical assessment and delivering evidence-based interventions within a multidisciplinary environment.
- Strong clinical reasoning and decision-making skills, with the ability to prioritise care and manage referrals effectively.
- Excellent communication and interpersonal skills, with the ability to build rapport and collaborate with patients, families, and service partners.
- Demonstrated ability to work effectively in a team, fostering collaboration and contributing to coordinated, patient-centred care.
- Knowledge of community resources and an understanding of care pathways that support seamless transitions from hospital to community.
- Current registration with AHPRA or relevant body
- Post Graduate qualifications (Gerentology or critical care -desirable)
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
