  • Temporary Part Time | Until October 2026

About the role

 

We are looking for experienced Community District Nurses who are enthusiastic about collaborating with people in their own homes and clinics, every day of the year.

 

You will provide clinical nursing services to adults receiving services in Community Health, Eastern Health at Healesville and/or Yarra Junction, utilizing both face-to-face in consumers residents and clinics models of care. The position will involve consulting closely with clients, families, and various stakeholders and providing care to:

  • People who need care immediately following their discharge from hospital
  • End of Life and /palliative care
  • People of any age who have a disability and who need nursing assistance to manage their healthcare needs
  • Wound Care
  • Diabetes management
  • Continence management
  • Medication management

This position is within the District Nursing Services and presents opportunities to work within a multidisciplinary team of Community Health. District Nursing Services prioritize low income and vulnerable families. 

 

 

About you 

 

 To be successful in this role you will be/have:

  • Registered nurse with the Nursing Midwifery Board of Australia (NMBA) via AHPRA
  • Advanced people skills with patients, families, and all health care professionals 
  • Experience in wound, catheter, and medication management in the community
  • Experience in complex care and knowledge of appropriate support programs
  • Minimum 3 years’ nursing experience, preferably in a community nursing role 
  • A strong commitment to high quality care
  • The ability to work as part of a diverse team.
  • Excellent communication and organisational skills 
  • Current Victorian Drivers Licence

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
