East Metro and Murrindindi Local Health Service Network (EMM LHSN)
The EMM LHSN is a collaboration between Alexandra District Health, Eastern Health, St Vincent’s Hospital Melbourne and Yea District and Memorial Hospital. Eastern Health is the lead agency for the EMM LHSN and therefore responsible for employing staff on behalf of the network.
The purpose of the EMM LHSN is to support consistent, equitable access to high quality care for patients across its geography, aiming to meet the healthcare needs of our population as close to home as possible. The EMM LHSN areas of focus are; access, equity and flow; our workforce; safety and quality; and our shared services.
To inform future activities, the EMM LHSN is undertaking a Network Clinical Services Plan (CSP) and a Mental Health and Wellbeing Collaboration Plan (MHWCP). Both pieces of work have a focus on partnering opportunities with local stakeholders and care providers such as Primary Health Networks, registered community health organisations, Aboriginal Controlled Community Health Organisations, aged care providers, community-based mental health and other providers.
About the role
The EMM LHSN Project Manager Community and Mental Health and System Integration Planning is accountable for leading the partnering‑opportunities component of Part B of the EMM LHSN CSP (non-admitted mental health and wellbeing services and partner care providers), and development of the Mental Health and Wellbeing Collaboration Plan, with a particular focus on cross‑sector collaboration that improves access, equity and flow, including for people experiencing mental illness.
The role leads the identification, development and activation of strategic partnerships across the four public health services, mental health and wellbeing services, Primary Health Networks, Aboriginal Community Controlled Health Organisations, community‑managed organisations and local government.
The position is employed by Eastern Health and works across the four health services. The role requires strong organisational skills, effective written and verbal communication and the ability to manage multiple priorities in a complex, multi‑organisational environment. Whilst the role will be primarily based at Eastern Health in Box Hill, it will be a requirement of the role of working with all four services in the East Metro and Murrindindi Local Health Service Network.
