About the Service

We are excited to launch a new in-reach rehabilitation model of care for stroke patients at Box Hill Hospital. This innovative service aims to optimise rehabilitation access by providing early assessment and intervention for patients who are awaiting transfer to rehabilitation, requiring rehab but not yet ready for subacute care, or those who can be discharged directly home with support. Working in partnership the award-winning acute stroke multidisciplinary team, the in-reach team will enhance patient flow and support high-acuity care by commencing timely, evidence-based rehabilitation interventions.

 

About the role 

Eastern Health is currently looking for passionate and experienced Grade 2 Physiotherapists to join our Allied Health Stroke team.  

On a daily basis you will live our Eastern Health values as you apply your skilled decision making supported by a clinical grade 3 and grade 4 neurological Physiotherapist as part of a high performing multidisciplinary team as you continuously innovate & deliver care to patients across an acute hospital environment. The role supports passion for provision of clinical supervision and ongoing development of yourself and the team, incorporating student education. This role carries engagement in multidisciplinary initiatives including service provision models, skill development, and leadership opportunities such as membership in Eastern Health working groups and committees. As a new expanded service this role has considerable opportunities for quality and innovative practice. 

 

About you 

To be successful in this role you will possess: 

Essential

  • Recognised tertiary qualification in relevant discipline. 
  • Current registration Australian Physiotherapy Board 
  • Eligibility for Membership of Professional Association. 
  • Experience in stroke/neurology clinical practice. 
  • Excellent team integration skills. 
  • Sound knowledge of community organisations and networks. 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
