About the role
Step into a dynamic Senior Clinician role within our Community Rapid Triage Team, where allied health expertise is at the heart of everything we do. You’ll work with genuine autonomy while being fully supported by a motivated team of allied health professionals & nurses with regular support of a Rehab Medical Consultant.
In this fast‑paced and engaging position, you’ll be the first point of contact for clients transitioning home after their Eastern Health hospital stay. You’ll draw on your allied health clinical reasoning to deliver rapid triage, functional assessment, goal‑directed planning, and clear discharge pathways that set clients up for success in the Community Rehabilitation Program, or divert them to more appropriate services.
Your skills in identifying early deterioration and escalating promptly will help prevent avoidable readmissions and ensure each client receives timely, safe, and effective care. Collaboration is central in this role—you’ll work closely with CRP therapists, medical staff, and key community partners to design holistic, person‑centred plans that genuinely support recovery and independence.

About you
To be successful in this role you will:
- Deliver targeted therapies and supports for clients, including complex presentations and frail clients.
- Apply strong clinical reasoning to holistic assessments and goal‑oriented plans.
- Identify risks early and implement effective management strategies.
- Collaborate seamlessly with referrers, CRP, Post‑Acute Care teams and key stakeholders.
- Drive innovation through evidence‑informed practice and quality improvement.
- Provide high‑quality, coordinated care across home and centre‑based settings.
- Contribute actively to a dynamic interdisciplinary team.
- Manage competing demands with excellent organisation and professional accountability.
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
