About the role
The primary role is to provide high quality assessment/discipline specific interventions, intensive case management services and assertive outreach support to people experiencing a mental illness utilising a recovery-oriented model of care.
Main responsibilities but not limited to:
- Clinical Case Manager undertaking case management and support the recovery for people with a lived experience of mental ill health within the BHMSTS.
- Work in partnership with people with a lived experience of mental illness and their families and carers to provide high quality assessment, intensive case management services and assertive outreach support using the Collaborative Recovery Model aiming to promote and facilitate recovery.
About you
To be successful in this role you will possess:
- Experience in supporting adults with the lived experience of mental ill health
- Ability to work as part of a diverse team and autonomously.
- Commitment to supervised practice, ongoing professional development and further developing skills in clinical mental health.
- A strong commitment to high quality care
- Excellent communication and organisational skills
- You must hold a valid driver’s license.
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
