About the role
The primary role is to provide high quality assessment, intensive case management services and assertive outreach support to people experiencing a mental illness utilising a recovery oriented model of care.
Main responsibilities but not limited to:
Clinical Case Manager undertaking case management and support the recovery for people with a lived experience of mental ill health within the BHMSTS.
Work in partnership with people with a lived experience of mental illness and their families and carers to provide high quality assessment, intensive case management services and assertive outreach support using the Collaborative Recovery Model aiming to promote and facilitate recovery.
Work collaboratively with the Consultant Psychiatrist, AMHS Rehabilitation Team Leader and Service Manager and other Senior Clinicians in MSTS providing leadership, ensuring high quality clinical standards, quality improvement and ongoing development of the Rehabilitation Stream.

About you
To be successful in this role you will possess:
Experience in supporting adults with the lived experience of mental ill health
A qualification in Social Work that has been approved by the Australian Association of Social Workers (AASW) for membership as a social worker, maintaining compliance with the AASW Continuing Professional Development Policy in order to meet the Eastern Health Allied Health credentialing standard
Ability to work as part of a diverse team and autonomously
Commitment to supervised practice, ongoing professional development and further developing skills in clinical mental health.
A strong commitment to high quality care
Highly developed interpersonal and leadership experience
A strong commitment to high quality care
Excellent communication and organisational skills
You must hold a valid driver's license
Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
