- The Aboriginal Health Team is a community based health service located in Healesville available for Aboriginal and Torres Strait Islander people and their families living anywhere in Eastern Health's catchment. The service adopts a holistic approach in line with cultural best practice, and provides a broad range of services which are available both on-site in Healesville and through home visits and outreach.

About the role
Lead a small team in providing home care to older Aboriginal and Torres Strait Islander community clients.
Position includes; rostering of clients and staff, planning and facilitating regular Elder activity outings, reviewing clients goals and care plans, client intake to home care services, liaising with clients and other services to provide timely and appropriate care, data entry and reporting tasks, general administration planning and supervision. Something different every day!
About you
To be successful in this role you will possess:
- Demonstrated advanced administrative skills
- Demonstrated ability to effectively lead and coordinate teams
- Sound knowledge of CHSP/HACC-PYP program and reporting
- Ability to lead and implement change
- Highly developed interpersonal and leadership experience
- A strong commitment to high quality care
- The ability to work as part of a diverse team
- Excellent communication and organisational skills
Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
