Community Engagement Team Leader – Community Health

About the role

 

 

The Team Leader Community Engagement provides operational leadership and professional support to a multidisciplinary team within Community Health, a key service of Eastern Health's Care@Home Program. The role leads a diverse workforce including: 

 

  • Intake clinicians
  • Health Promotion Officers 
  • Social Support clinicians 
  • Administrative staff 
  • Volunteers 

 

 

The Team Leader is responsible for ensuring high-quality, accessible and coordinated services while fostering a positive, collaborative and inclusive team culture. Working closely with the Community Health Leadership Team, you will drive service excellence, workforce development, quality improvement and achievement of key performance indicators. 

 

 

Key responsibilities

 

  • Lead and support a multidisciplinary community health team.
  • Promote high-quality, evidence-based and person-centred service delivery. 
  • Ensure services meet organisational, funding and performance requirements. 
  • Support workforce planning, supervision, performance development and staff wellbeing. 
  • Drive continuous improvement, innovation and service development initiatives. 
  • Build effective partnerships with consumers, community organisations and internal stakeholders. 
  • Contribute to operational management, programme reporting and service planning activities. 
  • Foster a culture of trust, accountability, inclusion and continuous learning. 

 

 

About You

 

 

  • A recognised Allied Health qualification. 
  • Current AHPRA registration or eligibility for practising membership with the relevant professional body. 
  • Demonstrated experience delivering high-quality, evidence-based health services. 
  • Strong leadership capability with a proven ability to motivate, coach and develop staff. 
  • Excellent interpersonal, communication and conflict resolution skills. 
  • Experience implementing quality improvement initiatives. 
  • A current Victorian Driver Licence. 
  • A commitment to person-centred care and Eastern Health's values.
