About the role
We are looking for experienced Community Nurses who are passionate about working with adults with chronic disease in the community.
You will provide clinical nursing services to adults receiving services in Community Health, Eastern Health at Healesville and/or Yarra Junction, utilizing both face-to-face and telehealth models of care. The position will involve liaising closely with clients, families, and various stakeholders and providing education and support, including Advanced Care Planning. This position is within the Health & Ageing Team and presents opportunities to work within a multidisciplinary team. Community Health services prioritize low income and vulnerable families.
About you
To be successful in this role you will be/have:
- Registered nurse with the Nursing Midwifery Board of Australia (NMBA) via AHPRA
- Highly developed interpersonal skills with patients, families and all health care professionals
- Experience in the area of chronic disease self-management
- Experience in complex care and knowledge of appropriate support programs
- Minimum 5 years’ nursing experience, preferably in a community nursing role
- A strong commitment to high quality care
- The ability to work as part of a diverse team
- Excellent communication and organisational skills
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
