About the role
We have an exciting opportunity for a Social Support Coordinator to join our Community Health team in the Yarra Valley, based primarily at Healesville Hospital and local outreach sites.
This role is ideal for someone who is passionate about social connection, community wellbeing and creative program delivery. You will design, coordinate and deliver engaging social support programs and group activities that help people stay connected, independent and well in their community.
Working closely with clients, carers and a broader multidisciplinary Community Health team, you will play a key role in developing and delivering flexible, person‑centred programs under CHSP and HACC‑PYP. You will be supported to trial new ideas, co‑design programs with participants, and continuously refine how social support is delivered to best meet the needs of people living in the Yarra Valley.
This is a unique opportunity to shape a modern, program‑based approach to social support in Community Health through combining creativity, autonomy and collaboration within a supportive and forward‑thinking team.
What you’ll be doing
- Planning and facilitating group‑based social support and community connection programs
- Working with clients to identify social needs, interests and participation goals
- Collaborating with allied health, nursing and community partners
- Supporting ongoing program review, co‑design and improvement
- Contributing to innovative approaches to social support in Community Health
About you
You’re someone who enjoys bringing people together and creating positive, welcoming spaces where people feel connected and supported. You’re creative, organised, and comfortable running group programs that make a real difference in people’s lives.
You’ll bring:
- A Certificate III in Aged Care (or equivalent) and twelve months’ experience (full time equivalent) as a Lifestyle Assistant Grade 1. Or certificate IV in Leisure and Health (or equivalent) and has six months’ experience (full time equivalent) as a Lifestyle Assistant Grade 1.
- Experience delivering group‑based social, community or wellbeing programs
- An understanding of how social connection supports health, independence and quality of life
- A creative and self‑motivated approach, with the confidence to run programs independently
- Strong communication skills and the ability to build rapport with a wide range of people
- Good organisational skills and the ability to manage your time and programs effectively
- Confidence in using digital tools (e.g. Microsoft Office) for planning, communication and documentation
Experience working in Community Health or collaborating with multidisciplinary teams is desirable.
