About the role

This is an exciting opportunity for dynamic and enthusiastic Grade 2 Social Workers to join the Community Rehabilitation Program (CRP) at Eastern Health.   

We strive to provide quality community based multidisciplinary care to support patients transitioning from bed-based services to their own homes. We are seeking client focused Social Workers who have a commitment to quality patient care and service improvement. The Social Work service involves direct practice and the delivery of a social work education program focused on improving client and carer wellbeing. 

 

This role will be predominantly based out of Yarra Ranges Health, however there is the opportunity of joining this role with another temporary part time role at Peter James Centre CRP for the right candidate, to create a temporary full time position until end of April 2027.

 

About you 

To be successful in the role(s) you will possess:

Strong commitment to high quality patient centred care in direct practice and group work. 

  • Clinical skills in managing a complex caseload using a client-centred approach 
  • High level communication skills, both written and oral, to enable development of a good rapport with clients and staff 
  • The organisational skills of a self-starter able to administrate a defined caseload 
  • Demonstrated ability to participate in innovative practice and quality improvement 
  • Deliver a high quality, accessible and coordinated service to our clients both in the client’s home, in the centre and in a group context. 
  • Ability to work collaboratively with other allied health disciplines to achieve client-centred goals 
  • Ability to be flexible and change work priorities as required in a fast paced, dynamic service 
  • Demonstrated confidence and interest in teaching Social Work students, in conjunction with the Community Rehabilitation Team. 

 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
