Our Employee Value Proposition is “Being a part of Eastern Health means belonging to a welcoming team of healthcare experts”.

This statement aligns with our promise to our communities, patients, consumers and staff that we will be HEALTHIER TOGETHER. Bolder than a vision for the future, our promise calls us to action. We know that working together is the only way we can achieve what is necessary for a healthier future.

About the role

We have an exciting part-time opportunity for a Grade 2 Social Worker to join our Care@Home Community Health program in a counselling role, based at Healesville and Yarra Junction (and potentially other outreach sites).  As part of the dynamic Health & Ageing Team you will play a key role in delivering individual counselling to children, young people and adults living in the community, with some potential for group work.

This position works within a multi-disciplinary team to deliver safe, evidence based therapeutic interventions to individuals, families and groups. Working from a child safety, family violence and trauma informed perspective, you will work with people from diverse and sometimes marginalised backgrounds. Counselling is provided in a face-to-face setting or via telephone/telehealth.

This position requires an advanced level of experience, knowledge and skills in counselling modalities, as well as liaison and consultation with other professionals, including Child Protection, Family Violence Services and Mental Health Services.

The successful candidate will be expected to work autonomously and develop quality improvement initiatives, whilst being supported by the senior clinician. You are expected to supervise and mentor social work students and provide secondary consultation to other members of the Community Health Team, as required.

This is a fantastic opportunity to work directly with people in the community, enhance your clinical skills, and work within a supportive and highly engaged team.

About you

To be successful in this role you will possess:

  • Compliance with AASW Code of Ethics and legal requirements of the Social Work profession.
  • At least 3 years of experience as a social worker

  • Demonstration of Professional Behaviours aligned with Eastern Health’s mission and values
  • Demonstrated ongoing commitment to professional development, education and supervision.
  • An understanding of confidentiality and professional boundaries
  • An understanding of the delivery of safe and effective care
  • An understanding of Community Health target populations
  • An ability to recognise, understand and report risk with sound judgement
  • Strong effective communication skills
  • Proven time management strategies and organisational skills
  • A willingness to continually learn and self-reflect
  • Proficient and confident with relevant workplace IT systems relevant to role including M365 applications (Microsoft office suite, Sharepoint etc)
