About the role

Opportunities exist within the Older Persons Community Team for passionate and enthusiastic qualified Case Managers with sound clinical knowledge and community mental health experience. You will provide recovery-based case management and have the ability to work autonomously as well as in a team environment.  This role is suitable for experienced Registered Psychiatric Nurses, Occupational Therapists, Psychologists or Social Workers. 

This position involves:

• Collaborate and engage with a multi-disciplinary team to provide discipline specific support to consumers
• Outreach visits to consumers
• Provide recovery orientated case management services
• Contribute to sustaining a positive working environment

Current opportunities available:

1. Casual Case Managers - based at Peter James Centre, Burwood East
2. Permanent, Full Time Case Manager - based at Ringwood
3. Permanent, Part Time Case Manager - based at Ringwood

What you’ll bring

• Degree in OT, SW, Nursing, or clinical psychology  
• Mental Health Experience
• Ability to work as part of a diverse team 
• Demonstrated leadership skills 
• Strong commitment to high quality recovery based case management
