About the role

 

The role of the Case Manager on a Continuing Care Team (CCT) is the assessment, co-ordination, treatment and support of adults with diagnosis of severe, chronic mental illness. You will be working with some of the most vulnerable members of our community and their families/carers to deliver safe and sustainable clinical interventions; supporting them to ultimately achieve better health and achieve their goals. About you 

 

To be successful in this role you will possess:

  • Minimum of two years working in public mental health
  • Current registration with AHPRA or eligible for AASW membership
  • Strong clinical and risk assessment skills
  • Strong documentation skills
  • A strong commitment to high quality care
  • The ability to work as part of a diverse, multidisciplinary team
  • The ability to manage an episode of psychiatric treatment from intake to discharge with continuity and to a deadline
  • The ability to communicate clinical information in an accurate, timely manner in an ISOBAR format
  • The ability to communicate and escalate safety and systemic issues with clinical and operational leaders
  • The ability to maintain a daily diary and continuously triage your work to meet clinical and operational demands
  • The ability to keep yourself and your colleagues safe, and be reliable under pressure
  • An active clinical supervision arrangement, or willingness to engage in same upon commencement
  • The ability to reflect on your practice and implement feedback given
  • Willingness to contribute to continuous improvement initiatives
  • Proficiency in Microsoft Office suite
  • Post Graduate qualifications in mental health are highly desirable

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.

Those that are offered and/or start between January and April do not require an up to date vaccination for Influenza to progress providing they are aware of its requirement later in the year. Those that are offered and/or start between May and December require the up to date Influenza vaccination prior to offer.
