About the role

 

Chandler House Continuing Care team is a dynamic multidisciplinary team providing case management for people adults with a severe mental illness. We work with consumers, their carers, families and other community agencies to identify and address their needs, coordinate their care and assist in recovery.

 

 

We are seeking a Clinician-Case Manager who would like to join our team in providing a highly responsive and flexible service to our consumers. This is an exciting opportunity for a clinician to contribute to our team's ability to provide a collaborative, person centred and recovery-oriented approach to our community consumers in a supportive environment. 

 

About you 

 

To be successful in this role you will possess:

 

  • Relevant registrations for your discipline/Current registration with AHPRA or eligible for membership of AASW 
  • Sound theoretical knowledge of clinical assessment and interventions in mental health. 
  • Demonstrated commitment to and ability to practice utilising the recovery approach in mental health treatment and support
  • Highly developed interpersonal and leadership experience
  • A strong commitment to high quality care
  • The ability to work as part of a diverse team
  • Excellent communication and organisational skills

 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
