About the role
Chandler House Continuing Care Team is a dynamic multidisciplinary team providing case management for people adults with a severe mental illness. We work with consumers, their carers, families and other community agencies to identify and address their needs, coordinate their care and assist in recovery.
We are seeking a Senior Clinician-Case Manager who would like to join our team in providing a highly responsive and flexible service for our customers. The Senior Clinician plays an essential role in our team's ability to provide a collaborative, person-centered and recovery-oriented approach to our community consumers in a supportive environment.
About you
To be successful in this role you will possess:
- Relevant registrations for your discipline/Current registration with AHPRA or eligible for membership of AASW
- Sound theoretical knowledge of clinical assessment and interventions in mental health.
- Demonstrated commitment to and ability to practice utilising the recovery approach
- Highly developed interpersonal and leadership experience
- The ability to work as part of a diverse team
- Excellent communication and organisational skills
