About the role
The Community Access Unit is a small yet dynamic team which processes referrals from internal and external sources for the Care@Home services such as the Community Rehabilitation Program, and Specialist Clinics including the Continence, Cognitive Dementia and Memory Service (CDAMS), Movement Disorders Program and the Falls & Balance Clinic.
This position will provide you with the opportunity utilise your clinical knowledge and skills in a phone-based triage service from receipt of referral, triage and acceptance to the clinic/s. You will expand your knowledge on the broad suite of services provided by Eastern Health's ambulatory care and community services and build-on your current clinical triage skills in a supportive environment.
Primarily you will be processing referrals, assessing the client's needs and referring/redirecting to the most appropriate service. You will liaise with key stake holders and consumers to facilitate appropriate referral information and pathways, and will work closely with various clinics to ensure up-to-date knowledge of eligibility criteria.
The Community Access Unit (CAU) is based at Eastern Health Wantirna with business hours of Mon-Fri 8:30am-5pm.
About you
To be successful in this role you will possess:(examples below)
- Current clinical qualification as a GD 2 Allied Health discipline or Registered Nurse
- Current AHPRA registration or relevant body
- Previous experience in an intake and triaging role
- Interest in specialist areas including CDAMS and Pain Management
- Demonstrated problem solving skills
- A strong commitment to high quality care
- The ability to work as paro fo a diverse team
- Excellent communication and organisation skills
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
