The Community Rehabilitation Programs offers short term goal directed rehabilitation aimed at improving client’s function and independence following a change due to a recent acute medical / health event. The multidisciplinary team provides clinical services to a wide range of clients who have a variety of conditions. The program is offered across four sites – Wantirna, Peter James, Angliss & Lilydale – or in the community.

 

 

About the role

 

 

We are seeking a dynamic and enthusiastic Allied Health or Nursing professional to take on the role of Team Leader. In this leadership position, you will work with the other Senior Clinicians and the Community Rehabilitation Manager to drive the professional and operational performance of the interdisciplinary teams and ensure key performance indicators and targets for the program are achieved monthly. This role is expected to lead and develop the operations of the program whilst providing leadership support and clinical advice to other Eastern Health services as required. 

 

 

About you 

 

 

To be successful in this role you will possess:

 

 

  • A relevant professional qualification in a health-related field with associated registration
  • Strong clinical and leadership experience, preferably in a rehabilitation environment 
  • Experience in managing multidisciplinary teams and implementing evidence-based practice 
  • Sound communication skills, including the ability to navigate complex, difficult or sensitive conversations 
  • High level of analytical thinking & skill involving research, benchmarking & resource identification including innovative service delivery models 
  • Experience with understanding of the broader health system, including community services and referral pathways  
  • A deep commitment to delivering & supporting high-quality, person-centered care through collaboration with others and development of individuals and the team 

 

 

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
